The process described by JP is not fully correct; there is no "redeem code" required. Check out this post on "Beyond The Bleeding Edge". But also read once again the caveat from Skype quoted at the beginning of this post. I am awaiting clarification of this issue for two reasons: (i) the work around is not a process available to the average Canadian Apple App Store end user and (ii) I can continue to remind Skype that Skype for iPhone has not been launched commpletely worldwide.

Workaround is quite easy.
Just switch to the US version of iTune’s AppStore (bottom of AppStore’s home page), create a US account using a redeem code (this will bypass the normally mandatory US credit card info) and download Skype.
